Enable patients to control their course to the greatest extent possible. F Fine-tune the pain plan as often as needed. (Agency for Health Care Policy and Research, 1994; Zerwekh, 2006) A complete assessment of pain includes the following: ❖ Pain history: When did the pain begin? What is the current medication regimen? ❖ Description: A patient’s description of the kind of the pain can help you better assess the most appropriate medication. Words can include dull, aching, gnawing, cramping, shooting, piercing, sharp, or burning.
Fact Patients with many chronic conditions live well for years on varying doses of opioids and other pain medications. While increased pain can be a sign that the disease is worsening, it does not foreshadow the end of life. Developing tolerance to opioids is generally slow, and all pure opioids (that is, not in combination with other non-opioids such as acetaminophen) have no maximum daily dose or “ceiling” on analgesic effect. In other words, pure opioids can be dosed as high as clinically necessary to achieve pain management.
